- A visa application form. //www.visaforazerbaijan.org.uk/
- A recent passport sized photograph,which must be glued to application form.
- Passport, which must be valid for at least three months after the expiry date of the Azerbaijani visa that is being applied for. There must be two blank pages facing each other.
- An original letter from the applicants’ employer on company letterheaded paper which must include the applicants’ name, the purpose of the journey, the name of the organisation to be visited, the number of entries and the duration of the visa being requested. The letter must state that the company accepts full financial responsibility for the applicant and be signed by someone senior to the applicant, or by the HR department.
- A letter of invitation addressed to the Embassy of Azerbaijan from the inviting company. The letter must be dated no later than 14 days before the applicants’ departure date and should bear their company stamp/seal. This letter can be printed out from an email or faxed.
- For non UK passport holders, a copy of two recent bank/credit card statements and/or utility bills from two different suppliers showing the applicant's name and current UK address must be provided.
